Well Visit Schedule

Unless your child is sick, children over the age of three are generally seen once a year for their annual check up. The easiest way to remember this is to schedule the visit around their birthday.

The scheduled intervals for well child exams:

  • Newborn visit (2-3 days after discharge from the hospital)
    • Jaundice check, length, weight, head circumference, Beyfortus (nirsevimab-alip) in-season RSV protection injection if needed
  • 1-2 weeks
    • Weight recheck
  • 1 month
    • Length, weight, head circumference, Edinburgh Postpartum Depression Screen (EPDS)
  • 2 months
    • Length, weight, head circumference, Dtap-IPV-HepB, Hib, Prevnar, Rotavirus (oral), EPDS
  • 4 months
    • Length, weight, head circumference, Dtap-IPV-HepB, Hib, Prevnar, Rotavirus (oral)
  • 6 months
    • Length, weight, head circumference, Dtap-IPV-HepB, Hib, Prevnar, EPDS
  • 9 months
    • Length, weight, head circumference, PEDS Developmental Screen
  • 12 months
    • Length, weight, head circumference, MMR, HepA, Prevnar, hemoglobin and lead (finger poke)
  • 15 months
    • Length, weight, head circumference, PEDS Developmental Screen, Dtap-IPV-Hib, Varicella, Pediavision (digital vision screen)
  • 18 months
    • Length, weight, head circumference, M-CHAT (autism screen), hemoglobin and lead (finger poke)
  • 24 months
    • Length, weight, head circumference, M-CHAT (autism screen), hemoglobin and lead (finger poke)
  • 30 months
    • Length, weight, head circumference, PEDS Developmental Screen, Pediavision (digital vision screen)
  • 3 years
    • Height, weight, blood pressure, MMR, Varicella, Pediavision (digital vision screen)
  • 4 years
    • Height, weight, blood pressure, vision, Dtap-IPV, Pediavision, hearing screen, PSC-17 (mental health screen)
  • 5-10 years
    • Height, weight, blood pressure, vision, PSC-17 (mental health screen)
    • 9 years – hemoglobin and cholesterol (finger poke)
    • Hearing screen every other year
  • 11 years
    • Height, weight, blood pressure, vision, hearing screen, Meningococcal MCV-4, HPV9 (1st dose), Tdap, hemoglobin (finger poke), PSC-17 (mental health screen)
  • 12 years
    • Height, weight, blood pressure, vision HPV9 (2nd dose), PHQ-9 (teen depression screen), PSC-17 (mental health screen)
  • 13-15 years
    • Height, weight, blood pressure, vision, hearing screen, PHQ-9 (teen depression screen), PSC-17 (mental health screen)
  • 16 years
    • Height, weight, blood pressure, vision, hearing screen, Meningococcal MCV-4), PHQ-9 (teen depression screen), PSC-17 (mental health screen)
  • 17 years
    • Height, weight, blood pressure, vision, hemoglobin and cholesterol (finger poke), consider Meningitis B, PHQ-9 (teen depression screen), PSC-17 (mental health screen)
  • Pre-College 
    • Height, weight, blood pressure, vision, PHQ-9 (teen depression screen), consider Meningitis B (if not done at 17 years)

Please note: the American Academy of Pediatrics recommends an annual flu vaccine for all children 6 months of age and older.
